Surgical Techniques for Brow Lift

Coronal Brow Lift (Forehead Lift)
An incision is made from ear to ear across the top of the head. The incision is placed just behind the hairline. The forehead is peeled down over the face. This gives excellent exposure to the muscles of the forehead and brow.
The forehead lift technique elevates all portions of the brow.
This plastic surgery is ideal for patients who want to lift the eyebrows and raise and shorten the forehead (i.e., it lowers the hairline in those with a high forehead). Therefore, it is more appropriate in patients with low foreheads.
It can be performed under intravenous sedation or general anesthesia.
The downside is the large incision, the potential for hair loss in the scar, and numbness on the scalp behind the incision.
Balding patients or those with thinning hair are not good candidates for approaches via the hairline due to increased visibility of the scar.
Supraciliary brow lift (Direct Brow Lift)
Requires removing a section of skin and underlying tissue above and following the length of the eyebrows. The surgical incision is created immediately over the brow hairs to camouflage the incisions.
The ideal candidates for a direct brow lift are those who experience functional blockage of the superior visual field attributable to low brow position and dark, thick (“bushy”) brow cilia.
No overcorrection is necessary with the direct brow lift due to the excellent mechanical advantage at this location.
The plastic surgery is easily performed under local anesthesia.
Transpalpebral Browpexy
Brow Pexy plastic surgery is performed through the same upper eyelid incision as in upper blepharoplasty.
It is a more limited procedure that addresses mild eyebrow ptosis. It doesn’t require raising the brow; instead, it needs support to prevent its descent over time.
The procedure allows fixation of the brow to the superior orbital rim to provide stabilization while also lifting the brow.
The sheer proximity of the upper eyelid to the brow bone makes the addition of a brow lift through an upper blepharoplasty very convenient with minimal additional risk and no extra incision.
It provides more subtle effects and makes it well suited for those who need just a little brow lifting or want a less invasive method.
The procedure is safe and enables satisfying aesthetic effects in the long term without leaving scars on the scalp.
It can be used in bald patients and does not cause alopecia.

Recovery - What to Expect After Brow Lift Surgery
When you awake from your brow lift, soft dressings will be in place. A small, soft drain may be placed to gently remove “wound fluid” under the skin for 1 – 3 days.
Patients typically experience very little pain after a brow lift, but it is common to feel slight discomfort and a sensation of tightness throughout the forehead. Pain medication is prescribed for you to take after the plastic surgery as needed.
You will be able to wash your hair in a day or two, and many patients return to work after 7-10 days or less.
Swelling and bruising are most common during the first 10 days or so after the plastic surgery and are mostly resolved after about 2 – 3 weeks.
Brow lift incisions tend to heal very well with good quality scars due to the excellent blood supply of the scalp and brow region, concealment of incisions within the hair, and avoidance of tension on the closure. It is crucial to minimize direct sun exposure to the forehead and face during healing to prevent permanent hyperpigmentation and sunburn.
As mild swelling and any bruising subside, visible improvements of the brow lift procedure will appear over a few weeks.
The final effect of a brow lift is a natural, lifted, more youthful and better-positioned forehead that will endure in the long term.
However, the natural aging process will continue after the plastic surgery. Proper skin care, living a healthy lifestyle, wearing sunscreen and additional non-surgical treatments can maintain the brow lift results last for years.

FAQs for Brow Lift
Can other surgical procedures be combined with a Brow Lift?
A brow lift alone improves the forehead and the upper eye region but does not specifically address the upper eyelids, the mid-face, cheeks or the neck and jawlines.
A brow lift is often complimented and further enhanced by a variety of complimentary facial plastic surgery procedures, including:
- Eyelid Lift (Blepharoplasty) to refresh and rejuvenate the eyes and remove excess, baggy upper or lower eyelid skin.
- Facelift to smooth wrinkles and correct aging in the cheeks and the mid-face.

How do I know whether a brow lift is right for me?

Will a brow lift make me look surprised or affect my ability to move my forehead?
This is a very reasonable question. A brow lift elevates your forehead by a moderate amount to give you a more youthful, refreshed appearance, but the results are designed to be subtle and natural. You will NOT look surprised after the procedure.
The forehead is affixed to prevent sagging, not to prevent it from moving. You will still be able to move your forehead normally after your brow lift plastic surgery.
