Blepharoplasty or botulinum toxin around the eyes

Eyelid surgery and botulinum toxin treat different tissue and are not alternatives to one another. This sets out what each corrects and when they are used together.

These two are often presented as competing options for the same problem. They are not. They act on different tissue, and if the wrong one is chosen the result is not a lesser improvement — it is no improvement at all.

What blepharoplasty does

Eyelid surgery removes or repositions excess skin and fat. It is the treatment for a hooded upper lid, for loose folds of skin, and for the bulge below the eye produced by protruding fat pads. Incisions sit in the natural upper lid crease or below the lash line, so they fall where the anatomy conceals them. Where an upper lid fold has begun to encroach on the field of vision, the operation is functional as well as aesthetic.

No cream and no injection will remove excess skin.

What botulinum toxin does

Botulinum toxin temporarily reduces the action of the muscles that produce dynamic lines — the creases at the outer corner of the eye that appear on smiling or squinting. Treatment takes minutes and there is essentially no downtime. The effect wears off over a period of months and the treatment is repeated.

It does not remove skin, and it does not address protruding fat.

Which applies to you

If the complaint is heaviness, a fold resting on the lashes, or bags below the eye, the answer is surgical. If the complaint is lines that appear on expression and soften at rest, the answer is injectable.

Age is a poor guide. Younger patients more often present with dynamic lines and older patients more often with skin and fat excess, but the pattern is sufficiently variable that the examination decides rather than the birth date.

Using both

The two are complementary and are frequently used together. Surgery corrects the structural problem; injections manage the dynamic lines that surgery does not touch, and continue to do so afterwards.

Duration and expectations

The surgical result is measured in years. The injectable result is measured in months and requires maintenance. Neither stops the ageing process — surgery moves the starting point, injections suppress a movement pattern for a period, and change continues from there in both cases.

Recovery

Blepharoplasty involves bruising and swelling that improve over the first weeks, time away from work, and a period without strenuous activity, with the final appearance emerging gradually. Botulinum toxin has no meaningful recovery.

The decision is made at examination, on skin elasticity, the position of the fat pads, the medical history and what you actually want changed.
